Membership Application Release

In consideration of being accepted into the membership of the Sitzmark Ski Club of Milwaukee, Inc., I certify that I am at least 21 years of age, and do hereby agree to abide by the rules, policies, and practices and the Code of Conduct of the Club (The Code of Conduct can also be found in the Membership Directory, and in the Sitzmark Ski Trip publication). Understanding that participation in the events of the Club carries a degree of risk and/or injury or death, I do hereby waive and release the Club, its officers, directors, agents, and members from any and all liability arising from injuries I may suffer as a result of my participation in said events and also agree to indemnify and hold it and them harmless from and against any and all damages, costs, and expenses incurred as a result of my actions. Further, I hereby grant full permission to any and all foregoing to use any
photograph, video tapes, motion pictures, recordings, or any other record of any ski club event for any legitimate purpose.
